Exploring Regenerative Medicine and Non-Surgical Adjunctive Therapies
By Axel Ruiz, MD, DABFP, CWS
Could You Be a Candidate for a Regenerative Medicine Evaluation?
Regenerative and non-surgical treatment options may be worth discussing if you are experiencing:
• Persistent knee or back pain
• Osteoarthritis of the knee, hip, shoulder, or spine
• Meniscus tears
• Ligament injuries, including MCL or LCL injuries
• Cartilage damage or degeneration
• Tendon inflammation or degeneration
• Persistent joint pain that has not improved with conventional treatment
• Joint stiffness or reduced range of motion
• Pain when walking, climbing stairs, exercising, or participating in sports
• Difficulty returning to normal activities because of chronic joint discomfort
Exploring Options Beyond Surgery
For appropriately selected patients, a comprehensive regenerative medicine program may incorporate exosome-based or stem cell-based biologic injections together with supplemental treatments such as shockwave therapy, therapeutic laser, red-light or near-infrared therapy, and rehabilitation.
Regenerative Biologic Injections
Exosome-based and stem cell-based products are being investigated for their potential roles in cellular communication, inflammatory modulation, and tissue-repair signaling. These approaches should be considered only after an appropriate medical evaluation and review of the patient’s diagnosis, imaging, and treatment history.
Shockwave Therapy
Extracorporeal shockwave therapy uses targeted acoustic energy and may be incorporated into a musculoskeletal treatment program to support local tissue response and address certain sources of chronic pain. It can be used as a supplemental modality when clinically appropriate.
Therapeutic Laser Therapy
Therapeutic laser uses selected wavelengths of light as a non-invasive adjunct. Depending on the device and clinical indication, it may be incorporated to support pain management, circulation, and cellular activity.
Red-Light & Near-Infrared Therapy
Photobiomodulation uses red or near-infrared wavelengths of light. Research is evaluating its effects on cellular energy production, inflammatory signaling, and pain pathways. It may be included as part of a broader rehabilitation program.
Why Combine Therapies?
Chronic joint and back pain can involve multiple structures and biological processes, including cartilage degeneration, inflammation, tendon or ligament dysfunction, muscle weakness, and altered biomechanics. For this reason, an individualized program may combine biologic procedures with supplemental shockwave, laser or red-light therapy and appropriate rehabilitation rather than relying on a single modality.
The Goal: Preserve Function and Explore Your Options
Not every painful joint requires surgery, and not every patient is a candidate for regenerative procedures. The first step is a comprehensive evaluation to identify the source and severity of the problem. Depending on the diagnosis, imaging findings, overall health, and degree of joint degeneration, an individualized non-surgical strategy may be considered before proceeding to major surgery.
BEFORE YOU DECIDE ON SURGERY, KNOW YOUR OPTIONS.
A personalized approach combining regenerative medicine, shockwave therapy, therapeutic laser, red-light therapy, and appropriate rehabilitation may offer selected patients another avenue to explore for persistent musculoskeletal pain.
IMPORTANT NOTICE: This material is for educational purposes only and does not constitute medical advice or guarantee clinical outcomes. Exosome products and many stem cell/cellular or tissue-derived products offered for orthopedic applications are investigational and are not FDA-approved to diagnose, treat, cure, or prevent osteoarthritis or other orthopedic diseases. Results vary, and no outcome can be guaranteed. These therapies are not substitutes for medically necessary surgery. A qualified medical evaluation is required to determine appropriate treatment options.
