Every day in my Sarasota clinic, I see patients who are frustrated by chronic pain. What many don’t realize is that the root cause isn’t actually in their back, hips, or knees— it starts the moment their feet hit the ground. Every step you take places up to three times your body weight of force on your feet. When your structural alignment is slightly off, that repetitive force travels upward, stressing your entire body.
While over-the-counter shoe inserts offer generic cushioning, they treat every foot as if it shares the same shape. In reality, your feet are as unique as your fingerprints. When I prescribe custom orthotics, I am designing medical devices engineered to correct your specific biomechanical imbalances, redistributing pressure and restoring your natural alignment.
To show you what a difference this makes, let’s look at three entirely different structural challenges I frequently treat.
Case 1: Overcoming True Structural Imbalance (The 3cm Leg Length Discrepancy)
Not long ago, a fit, male athlete came to see me dealing with a structural leg length discrepancy—his left leg is 3 centimeters shorter than his right. Without intervention, his body had to constantly compensate. His pelvis tilted, his spine curved to keep his torso upright, and his left foot drastically overpronated (rolled inward) just to reach the ground. This resulted in severe wear-and-tear, leading to chronic lower back pain, hip bursitis, and Achilles tendonitis.
Generic heel lifts simply cannot adequately address a 3cm deficit without destabilizing the shoe or failing to support the arch during high-impact sports.
My Custom Solution: I designed a custom orthotic that integrates a precise structural lift directly into the bed of his left insert, carefully calculated alongside his running mechanics. By bringing the ground up to meet his left foot, we leveled his pelvis and aligned his spine. It provides a stable, neutral platform that eliminates destructive bodily compensations, allowing him to run and compete symmetrically without pain.
Case 2: Shock Absorption for Rigid Structures (The Elderly Woman with High Arches)
On the opposite end of the spectrum is a condition called pes cavus, or abnormally high arches, which I frequently see in my elderly patients. A high-arched foot is inherently rigid. Because the arch does not collapse to absorb shock when hitting the ground, impact forces concentrate heavily onto two small areas: the heel and the ball of the foot.
For one of my sweet elderly patients, this rigidity made walking on hard surfaces incredibly painful. It led to severe calluses, burning pain in the forefoot, and balance instability that increased her risk of falls.
The Problem: Standard flat shoe beds leave a hollow void beneath a high arch, providing zero support and forcing the heel and forefoot to bear 100% of the weight.
My Custom Solution: I molded custom orthotics to perfectly mirror the unique contours of her high arch, expanding the surface area bearing her weight. This immediately shifted pressure away from the painful areas, redistributing it evenly across the entire sole. Specialized, medical-grade materials restored the shock absorption her feet naturally lacked, significantly improving her stability and confidence when walking.
Case 3: Guiding Developing Foundations (The 9-Year-Old Girl with Flat Feet)
Pediatric biomechanics require a very different, careful approach. I recently treated a 9-year-old girl with symptomatic pes planus (flat feet), which means her foot structure is overly flexible. When she stands, her arches completely collapse, causing her ankles to roll inward and her knees to knock together.
Her parents told me she often complained of leg pain, tired out quickly during sports, and frequently tripped. At age 9, her skeletal structure is still growing and highly adaptable. Leaving severe overpronation uncorrected can alter the development of her adult gait and posture.
My Custom Solution: The custom pediatric orthotics I prescribed act as a dynamic, supportive mold during these crucial developmental years. They feature a deep heel cup to stabilize the heel bone and a calibrated arch support that prevents the foot from collapsing inward. This keeps her ankles, knees, and hips in proper alignment, eliminating the muscle fatigue and leg pain so she can just focus on being a kid and enjoying active play.
